We will need the following information from you to begin the dispute process.
- What are the date and dollar amounts of the charge in question?
- What is the merchant’s name on the statement?
- Is the card in your possession?
- Does anyone else have access to your PIN?
- If the card was lost or stolen, when did this happen, and how?
- What is the last valid transaction? When?
- Why is the debit charge erroneous? What is the reason for the dispute? (Ex. charged twice for the same item, unauthorized charge)
- If an unauthorized transaction, have you ever done business with this merchant?
- If charges were with the incorrect amount, do you have supporting documentation? (Ex. copies of email communication with the merchant)
